Dental care
  • Year 1: 50% of first $1,150 with $575 annual maximum per person
  • Year 2+: 80% of first $400 and 50% of next $860 with $750 annual maximum per person
  • Recall visits every 9 months per person
  • Includes fillings, cleanings, scalings, examinations, polishing and select extractions
  • Excludes oral surgery, periodontics, endodontics (such as root canal), orthodontics, crowns, bridges and dentures
Vision care
  • $250 per person every 2 years towards prescription lenses, frames, contact lenses and laser eye surgery
  • $60 per person every 2 years towards optometrist fees
  • Preferred Vision and Hearing Services (PVS) discounts of up to 20% on lenses, frames, contact lenses and laser eye surgery at participating optical retailers (visit www.pvs.ca for more details)
Extended health care
  • $250,000 lifetime maximum per person
  • Paid after government health plan maximums are reached where applicable

Acupuncturist, Chiropodist, Chiropractor, Naturopath, Osteopath, Podiatrist, Registered Massage Therapist – maximums for each:
  • $25 per visit per person
  • 20 annual visits per person

Physiotherapist – annual maximum:
  • $250 per person
Psychologist, Psychotherapist – annual combined maximums:
  • $80 for first visit
  • $65 for subsequent visits

Hearing Aids
  • $400 maximum per person every 4 years towards purchase or repair

24-Hour Monitoring Service
  • 6 months per person every 3 years of Lifeline® Personal Response Service
Accidental Dental
  • $2,000 annual maximum per person

Health Service Navigator®
  • Medical coordination services
  • Second medical opinions from world-class hospitals
Travel
  • $5 million per person in emergency health coverage for the first 9 days of each trip, with a $100 deductible per claim
  • Coverage may be limited or excluded for any illness or condition that first manifested itself within the 9-month period immediately preceding each departure date
  • Coverage ends at age 70
Accidental Death and Dismemberment (AD&D)
  • Up to $25,000 in protection for adults under age 65
  • Up to $10,000 in protection for children and adults age 65 and over
Survivor Benefit
  • Waives premiums for 1 year after the death of an adult insured
  • Available 1 year after policy effective date

For Vision Care and Hearing Aid benefits, one year is defined as a Benefit Year (the 12 consecutive months following the incurred date of the claim). For all other benefits, one year is defined as an Anniversary Year (the 12 consecutive months following the Effective Date of the Policy and every 12-month period thereafter).
